- help you set goals for a healthier lifestyle
- coordinate your appointments and services.

People who live in the Latrobe local government area and have a long-term health condition like diabetes, a heart or a lung condition can see the chronic disease care coordinators.
If you use more than one service at Latrobe Community Health Service, seeing a care coordinator could also be helpful.
To attend the Better Health Self-Management course, you must have a long-term health condition and live in the Latrobe local government area. Your partner, carer, or other interested family members can also attend the course.
You must be able to commit to attending the six-week program, as each week builds on the knowledge of the previous sessions.
There is no charge to see the chronic disease care coordinator.
